Regarding payment
Payment is required at the time of services rendered. We accept cash payments, personal checks, Visa and Mastercard.
Regarding insurance
- Our office has Out of Network status with most insurances
We have in-network status with ConnecticareCare only
- You can call your insurance companies directly to check on the out of network benefits for your individual insurance plan
- Our office will provide claim submission services for Anthem Blue Cross/Care, Aetna, Cigna, and Oxford at no additional charge to patients
- In most cases, you can expect to receive your out of network benefits within 2-4 weeks of your visit
Cancellation/Rescheduling policy
Please be aware there is a 48-hour cancellation policy. Appointments cancelled with less than 48 hours notice will be charged at half of the full cost. If an emergency occurs and you cannot make an appointment this policy will not be in effect the first occurrence. A credit card will be requested when you book your appointment to cover these charges.
